Use this form to update the accountholder contact information or communication preferences. For security purposes, confirmation of any account changes will be sent to both the existing contact information on file and the updated contact details.

Important Notes:

- Beneficiaries can only update account information if the original accountholder is deceased (documentation required).

- Power of Attorneys (POAs) may only make changes to accounts they have been previously designated to by the accountholder. To assign a POA, the accountholder must complete STRATA's Power of Attorney Designation form.
